Regional Case Manager (Portage La Prairie)

Turning Leaf is a non-profit charitable organization dedicated to providing helpful services to those experiencing intellectual challenges and mental illness. This is an exciting opportunity for someone to utilize their experience and skills to move the organization to the next level!

Reporting to the Senior Residential Case Manager, the Regional Case Manager is responsible for the operations of all services provided to all Turning Leaf participants within the region. Specifically, you are responsible for overseeing the service, management and operation of the residential facility, and ensuring that program objectives and individual support contracts/support plans are delivered and developed.

Duties and Responsibilities

Leadership
--Demonstrate the agency values, ethics and encourage regional staff to embody them both in the completion of their duties and when representing the agency in the community
--To serve as role model, communicator and implementer of policy and procedure
--Engage the community in such a way so as to create opportunities for participants; positively represent the agency and enhance the agency’s reputation
--Represent the agency within the system team (e.g., amidst collaterals, funders, family, etc.)

Supervision
--Oversee the scheduling of staff coverage in the group residences and in community on a day to day basis, and arrange relief staff as necessary
--In collaboration with Human Resources, participate in the selection of staff
--In conjunction with Human Resources, conduct orientation / onboarding training as needed
--As needed, prepare and provide training to regional staff
--Maintain services standards so as to remain compliant with Residential Licensing
--Conduct weekly review of house budgets to ensure compliance with spending guidelines and checks house monies weekly

Reporting
--To report any unusual occurrences to the Senior Residential Case Manager and when directed, a representative of all appropriate agencies (e.g., Family Services and/or Probation Services)
--To inform the Senior Residential Case Manager of any change in their status and disclose any conflict of interest
--Weekly comprehensive summary report and participant monthly progress notes to be submitted and reviewed by the Senior Residential Case Manager
--Formal standardized written incident reports on the participants’ incidents as defined by the Province of Manitoba to be submitted to the Senior Residential Case Manager within 24 hours of the incident; verbal notice to the Senior Residential Case Manager of incident to occur within 6 hours of the support worker’s knowledge of the incident
--Completes the fine option/ community service program with probation services

Operations
--Ensure the efficient operation of the residence and Residential support contracts
--Aid in coordinating services with the appropriate employment supports services/school/day programs, services coordinators, and other consultants
--Implement behavior management strategies, training, and support in a non-intrusive, non-threatening, non-manipulative, supportive manner, in the least restrictive manner as is possible
--In consultation with Support Team, develop an individualized support plan for the participant, using available data pertaining to the participant
--Maintain an awareness (and record) of the participants’ known diagnoses and symptomatic behavior. Support plans to reflect the participant’s mental health condition and intellectual abilities
--In conjunction with support team, engage participant in a process of goal setting, planning and acquisition as it pertains to living safely and independently in the community
--Ensure there is structured programming throughout the day and evening to support the development of skills pertaining to healthy and safe independent living skills

Essential Requirements
--Recent Criminal and Adult Abuse Registry Checks
--Post-secondary degree or certificate related to human services
--Minimum of three years’ experience in providing service to intellectually challenged adults and those experiencing mental illness
--Program development experience
--Knowledge of provincial licensing compliance
--Ability to operate the region within budget
--Valid CPR and First Aid certificate
--Valid driver’s license with access to vehicle
--Self-starter and proactive approach in resolving problems and issues
--Excellent oral and written communication skills
--Several years’ Supervisory experience
