Jordan’s Principle Case Manager

Primary Duties – The Jordan’s Principle (JP) Case Manager has the primary responsibility of community-based Jordan’s Principle Service Coordinators who will work directly with Indigenous children and families to assist with the navigation of the whole range of federal and provincial services/supports (e.g., education, health, and social) and submitting Jordan’s Principle requests on their behalf. The focus will be to coordinate access to products, services and supports for both children in care and children not in care of the agency and to ensure services and supports are accessed without delay.
Specific Duties – without limitation to the above primary duties, a JP Case Manager performs the following specific duties:
1. Familiarize self with all Department of Indigenous Services Canada documentation related to the Canadian Human Rights Tribunal and Jordan’s Principle;
2. JP Case Manager will coordinate and plan– service workflow processes and make recommendations to modify the process to the Program Co-ordinator;
3. JP Case Manager will apply agency service coordination and workflow process from intake, ongoing case management and discharge in conjunction with agency mandated and non-mandated programs;
4. Manage service-related data collection processes as defined by senior management;
5. Work in collaboration with assigned finance department staff to ensure that all financial claims related to Jordan’s Principle are processed and managed according to agency policy and procedures;
6. JP Case Manager will actively participate in internal, external, local, regional or committees/groups in support of coordinating programs and services for members of Dakota Plains Wahpeton Nation;
7. Participate in internal or external committees as required or requested;
8. Liaise and work effectively with the community served, service providers, collateral agencies, organizations, and elders Administration;
9. Complete administrative functions and adhere to all Agency policies, procedures and relevant practices;
10. Formalize monthly reports in collaboration with the Program Co-ordinator that report Jordan’s Principle service coordination service review, analysis and referral activity and service gaps related to mandated and non-mandated service areas of the agency;
11. Ensure confidentiality and safe keeping of all Agency documents and records;
12. Develop and maintain work files that are accurate, up-to-date and concise;
13. Work in compliance with the Occupational Health and Safety Act and any other legislation;
14. Prepare and submit monthly reports, attendance records and travel expense claims;

  1. Follow the Agency’s Human Resource, Finance and other Policies and Procedures in the performance of duties Jordan’s Principle Case Manager;
  2. The terms of the Dakota Plains Wahpeton Nation’s Human Resource Policy (“the Human Resource Policy”) are incorporated as part of the within Job description and the employee is required to become familiar with the content of the Human Resource Policy.
  3. Complies with the terms of the Human Resource Policy as may exist from time to time and which terms are not otherwise in conflict with the within Job Description;
  4. Adheres to the Codes of Conduct as contained in the Human Resource Policy as may exist from time to time;
  5. Encourages and fosters a team spirit within the Dakota Plains Wahpeton Nation and its operations generally. Qualifications: The qualifications of the JP Case Manager determined in the absolute discretion of Chief and Council depending upon the needs of Dakota Plains Wahpeton Nation at the time. The following attributes are assets:
  6. Has successfully completed any one of the following: a. A baccalaureate degree in nursing from a Canadian university or a recognized equivalent; b. A certificate of a diploma in community health nursing from a Canadian university or a recognized equivalent;
  7. Two (2) years’ experience coordinating and/or managing social programs and services;
  8. One (1) year direct service experience with children and families;
  9. Experience working with aboriginal people, organizations, and communities;
  10. Equivalent combination of education and experience may be considered;
  11. Knowledge of local services available to children and families;
  12. Knowledge of CWIS and CIMS;
  13. Knowledge and understanding of the Child Youth and Family Services Act;

  24. Ability to work with confidential and highly sensitive and personal information;
  25. Must provide a clear Police Records Check with Vulnerable Sector Check (Child Abuse Registry);
  26. Must have a valid Manitoba Driver’s License, access to a vehicle and be able to travel;
  27. Working experience with Wahpeton Nations with an understanding of the particular issues encountered by Wahpeton Nations;
  28. Knowledge of traditional cultural heritage and language is an asset.
