Reaching Home Case Manager
Full time term until March 31, 2025 - Starting at $20.00/hr Monday to Friday - 8:30 a.m. to 4:30 p.m.

PCRC is currently seeking qualified candidates to fill the new role of the Reaching Home Case Manager. This position is funded by the Government of Canada’s Reaching Home: Canada’s Homelessness Strategy. Reaching Home: Canada's Homelessness Strategy is a community-based program aimed at preventing and reducing homelessness across Canada. The main objective of this program is to assess homelessness in Portage la Prairie and connect individuals/families to suitable housing, supports and services.

Conditions of Employment:
• Must be legally entitled to work in Canada
• Satisfactory Criminal Record Check (with vulnerable sector check), Child Abuse Registry Check, Adult Abuse Registry Check and Prior Contact Check
• Valid Driver's license, access to a vehicle and the ability to travel within Portage la Prairie and surrounding areas

• Post-secondary degree/diploma in social sciences or human services desired. Other combinations of education and experience may be considered
• Experience in case management in the social services field with an emphasis on case management which includes assessment, service planning, evaluation, follow up and review and closure
• Excellent interpersonal skills with the ability to engage and establish working relationships with individuals/families, local landlords and community organizations and agencies
• Excellent ability to manage conflict
• Excellent time management and organizational skills with the ability to prioritize competing workload demands and meet time sensitive deadlines
• Excellent problem solving and decision making skills
• Ability to work with a diverse population
• Excellent written communication skills, with experience writing clear, concise documents and reports for a variety of audiences
• Experience writing policies and procedures and adhering to them
• Experience report writing and determining measurable outcomes
• Ability to work in a fast paced team environment

Reporting to the Reaching Home Coordinator of the PCRC, the Reaching Home Case Manager will establish and maintain effective working relationships with individuals/families, advocates, landlords and service providers/agencies often under difficult circumstances. The successful candidate will promote the Reaching Home program to the community to build awareness, leverage funding and build relationships with relevant stakeholders. This position requires effective case management skills incorporating sound judgement and administrative management as well as workload management using Housing First methodology and practices.
